COMMITMENT POLICIES

I understand that enrollment in private lessons at The Music Box commits its students to their weekly scheduled lesson. Regular attendance supports the development of skill and repertoire, and promotes student preparation for performance opportunities offered throughout the year.

TUITION POLICIES

I understand that the set monthly tuition rate at The Music Box is based on the average of lessons offered throughout the year. Holiday lesson closures are not billed for in the annual average. I understand that tuition is automatically paid on the 1st of each month and considered late after the 10th of each month. Payments received after the 10th of each month are subject to a late fee of $25. Services will be discontinued for any unresolved tuition balances on the 15th of each month.

WITHDRAWAL POLICIES

I understand that membership at The Music Box commits its students to their instructor’s weekly private lesson calendar. Memberships auto renew monthly, until notice of withdrawal is received. To be removed from the instructor’s private lesson calendar, the academy must receive notification by the 4th of the student’s last month. Withdrawal requests received after the 4th of the current month will be eligible for the next withdrawal period, and the student will automatically be enrolled on the instructor’s private lesson calendar for the month following the current month.

*If the 4th falls on a weekend or holiday, withdrawal requests can be provided via email or by leaving a voicemail at the studio. All withdrawal requests received by the monthly deadline of the 4th will be honored if the studio is closed that day.

ABSENCES & MAKE UP POLICIES

I understand there are no refunds, tuition deductions, or rescheduled lessons provided for student absences. If the instructor cancels a private lesson, the student’s account will be credited for the amount of tuition related to the canceled lesson(s). In the event of a student absence, the student has the option to request a LessonMate. * A LessonMate is a video make up lesson supplement to support progress in the event of an absence. LessonMates are completed by the instructor during the missed lesson time.

LESSONMATE REQUEST POLICY

Deadline for Afternoon Lessons: I understand that a request for the receipt of a LessonMate must be placed by 2pm on the day of the scheduled lesson.

Deadline for Morning or Saturday Lessons: I understand that a request for the receipt of a LessonMate must be placed by 9am the day of the scheduled lesson.

Requests placed after the provided deadlines cannot be honored.

SUBSTITUTE POLICIES

I understand that the instructor reserves the right to send a substitute instructor from The Music Box’s approved substitute list in the event of his/her absence. The substitute instructor will complete requested LessonMates in the instructor’s absence.

INCLEMENT WEATHER POLICIES

I understand that in the event of inclement weather, The Music Box may make decisions independently of local school districts. In the event a studio closure is necessary for student and staff safety, students will receive a LessonMate from their instructor. Please anticipate receipt within 72 hours in the event of power outages.

TARDY POLICY

I understand that it is my responsibility to communicate a late arrival with the front desk. The instructor will check the lobby and corresponding ZOOM room for students up to 10 min after the start of the scheduled lesson. I understand the lesson will be forfeited if I have not arrived or notified the front desk of tardiness within 10 minutes of the start of the lesson.

CHILD SAFETY POLICIES

The Music Box has an OPEN DOOR policy. Parents are welcome to sit in on lessons at any time and encouraged to check in periodically. For added student safety and parent enjoyment, viewing monitors are on display in the lobby. To ensure your child’s safety, children will not be allowed to wait in the reception area unattended. Please make sure your child makes it to their lesson and the instructor is present. The Music Box faculty and instructors are unable to supervise children waiting in the reception area before and/or after classes/lessons.

INSTRUCTOR CHANGE POLICIES

In the event of an instructor’s departure, The Music Box will notify the student/student’s parent of the replacement instructor and the date of instructor change.

PHOTO & VIDEO RELEASE

I give permission to The Music Box, LLC. to use my child’s name and photographic likeness in all forms and media, whether electronic, print, digital, video or electronic publishing via the Internet for advertising, trade, and any other lawful purposes.
